TIME: 5:30 pm - 7:30 pm

• Register for the 2019 season
• Turn in Physical Forms
• Turn in Student Profile Form
• Complete MTJM & SYF Waiver and Release Form (see below steps to successfully e-sign your forms)
• Make a payment 

MTJM & SYF Electronic Waiver & Release Form 
You can now review & complete the MTJM & SYF Waiver & Release forms online via e-signature! 

E-signatures will be sent via email to all registered participants beginning 7/10/2019. When received, please follow the below steps to successfully e-sign your forms:

• First Email to be completed by Parent/Guardian
• Second Email to be completed by Participant

If you have registered your child for the 2019 Football & Cheer season and have not received an email to e-sign the MTJM & SYF Waiver and Release Forms, please click HERE

EGYSF Football & Cheer Scholarships

The Elk Grove Youth Sports Foundation is a nonprofit organization that provides annual scholarships to qualified families.

You can apply for an EGYSF scholarship between 4/1/2019 - 6/30/2019.

Please follow the below steps to complete your application:

• Complete a Jr. Mustang registration form for the 2019 season by clicking HERE
   
• Complete a EGYSF Scholarship Application by clicking HERE

What happens after I apply?

The Jr. Mustangs will notify you if/when your scholarship application has been approved.

Please do not contact the EGYSF to follow up on your application. The EGYSF prefers to handle all communication regarding submitted applications directly with the Jr Mustang Board.
